Qian-Kun-Nin is a Chinese herbal formulation considered to have anti-infection, anti-tumor and immuno-enhancing properties. Data from previous investigations showed that Qian-Kun-Nin causes HIV growth inhibition and immunomodulation in vitro, suggesting that this formula has the ability to inhibit HIV and modulate impaired immune functions in humans. We conducted this pilot study to evaluate the anti-retroviral and immunological enhancing effects of this formula on HIV positive subjects. Eight subjects completed the study, receiving oral Qian-Kun-Nin capsules for 24 consecutive weeks in a single blind design. Compared to baseline level, the plasma virus load decreased significantly at the end of week 12 (p < 0.01) and week 24 (p < 0.01), respectively. Four weeks after cessation of Qian-Kun-Nin treatment, plasma virus load was still significantly lower compared to baseline (p < 0.01). Blood CD4 cell counts were increased significantly at the end of the 12th week compared to the baseline level (p < 0.01). No adverse effects were observed, and no significant side effects were recorded in any subjects. These data suggest that Qian-Kun-Nin has therapeutic potential in the treatment of HIV positive patients.

Vandellia cordifolia (V. cordifolia) used for treatment inflammation in traditional Chinese medicine was selected for immunopharmacological activity test. The effects of V. cordifolia extracted fractions on human mononuclear cells (HMNC) proliferation were determined by tritiated thymidine uptake. The results indicated that VC-ME fraction suppressed HMNC proliferation activated with phytohemagglutinin (PHA) and stimulated cell cycle progression was arrested at the G0/G1 stage. The inhibitory mechanisms may involve the blocking of interleukin-2 (IL-2) and interferon-γ (IFN-γ) production, since VC-ME suppressed IL-2 and IFN-γ production of HMNC in a dose-dependent manner. Therefore, it is suggested that immunomodulatory agents are contained in V. cordifolia.

We investigated the protective effect of Yang-Gan-Wan (YGW, Pro-Liver pill), a Chinese herbal remedy, against liver apoptosis (programmed cell death) induced by anti-Fas antibody (Jo2). Mice were pretreated daily with 200 mg/kg YGW for 14 day before treatment with 10 μg/20 kg body weight Jo2. YGW significantly reduced the elevated activity of alanine aminotransferase (ALT) and also reduced the elevated activity of sorbitol dehydrogenase (SDH) produced 4 hrs after treatment with Jo2. YGW also increased the survival of mice treated with Jo2 by about 1 hr in 11 out of 16 mice (69 %). Most of the mice died 9 hrs after Jo2 injection. This study demonstrates that YGW has protective effects against liver apoptosis induced by Jo2.

Manganese superoxide dismutase (MnSOD), an inductive antioxidant enzyme, can protect cells from oxidative injury to the mitochondria. The elevation of MnSOD activity in cells can effectively prevent many diseases associated with oxidative stress. Polysaccharide Krestin (PSK), a kind of protein-bound polysaccharide extracted from Coriolus versicolor, is used as an immune response modifier in anti-tumor therapy. We have previously found that PSK could alleviate the oxidative injury that oxidized low density lipoprotein (Ox-LDL) brought to monocytes/macrophages, and therefore had some preventive or therapeutic effect on atherosclerosis. In order to find out if the effects of PSK were associated with the alteration of antioxidant enzymes, we investigated its effect on MnSOD activity and gene expression in mouse peritoneal macrophages. The results showed that PSK could enhance SOD activity and increase the contents of MnSOD mRNA in mouse peritoneal macrophages. Furthermore, the induction of MnSOD by PSk could be blocked by cycloheximide and actinomycin D.

The effects of Yukmi (Decoction of six plants including rehmannia), an herbal formula, were studied on liver oxidant damage induced by paraquat (PQ) administered intravenously in the senescence accelerated mice (SAM-P/8). The activities of superoxide dismutase (SOD) and catalase as two major antioxidant enzymes and lipid peroxidation levels were determined for six days. Data show that the activities of hepatic SODs and catalase were increased by oral administration of Yukmi extracts following PQ pretreatment. Herbal medicine effectively blocked the PQ-induced effects on liver malondialdehyde (MDA) levels. For the histopathological changes in SAM-P/8 liver, Yukmi extracts inhibited PQ-induced damage to the hepatic mitochondria and their membranes. Data suggest that Yukmi extracts may be useful in protecting against oxidative damage.

Hepatitis C virus (HCV) infection frequently causes chronic hepatitis, which is linked to the development of liver cirrhosis and hepatocellular carcinoma. Most physicians who practice Kampo medicine in Japan have observed that Kampo medicine can be as effective as interferon therapy in the treatment of chronic hepatitis C. In the present study, to evaluate the effect of Kampo medicine on chronic hepatitis C, clinical treatment was assessed in short-term and long-term study, and it was shown that Ninjin-yoei-to (Formula ginseng compositae: TJ-108 was very effective. Therefore, to find the most active herbal component of TJ-108, in the treatment of HCV, Citrus Unshiu Peel, Schisandra Fruit, and Polygala Root, which are specific to TJ-108, were screened using an in vitro HCV infection model. Among the three herbs, Schisandra Fruit was found to be most active. In the next step, Gomisin A, an active component of Schisandra Fruit, was studied using an in vitro model with MOLT-4 cells and an animnal model of immunologically induced acute hepatic failures. It is concluded that the therapeutic effect of TJ-108 on chronic hepatitis C is from the inhibitory effect on HCV infection, and also from the protective effect on immunological hepatopathy of Schisandra Fruit and its lignan component, Gomisin A.

The hepatoprotective effects of Tao-shang-tsao, Ixeris laevigata SCH.-BIP. var. oldhami KITAM., were studied on cholestatic hepatitis induced by α-naphthylisothiocyanate (ANIT, 100 mg/10 ml/kg, in olive oil, i.p.) and acute hepatitis induced by carbon tetrachloride (20% CCl₄/olive oil, 1.5 ml/kg, i.p.) in rats by post-treatment with the crude methanolic extracts of I. laevigata (0.3, 1.0, and 2.0 g/kg) orally in the therapeutic model. Hepatoprotective activity was monitored by estimating the serum transaminases concentrations and histopathological changes in the livers of experimental rats. It was found that the I. laevigata extract significantly decreased the acute elevation of serum transaminases by biochemical examination. According to pathohistological studies in the liver of experimental rats, the crude I. laevigata extract ameliorates the central necrosis, fatty change or proliferation of bile duct epithelium focal necrosis caused by ANIT or CCl₄-induced hepatitis rats.

Our previous report first showed evidence that polysaccharides isolated from ginseng leaves obtained from Jilin, China possess luteotropic activities. In this study, we made further investigations on the root and flowers of Korean ginseng by means of the same bioassay system described briefly as follows. Frozen-thawed bovine luteal cell (1×10⁵ cells/ml/well) in M199 were incubated in 24-well culture plates at 37°C in a 5% CO₂ incubator. Ten μl of tested drugs with 1, 10 and 100 μg/ml were added into each well. Afer 4- and 24-hr incubation, the media were harvested and assayed for progesterone by an enzyme immunoassay. The production of progesterone from cells is the indicator for evaluating the action of tested drugs. Results showed that hot water extracts of ginseng flowers (GF-1) with 10 to 100 μg/ml significantly increased progesterone production, whereas those from ginseng root(GR-1) could not. Crude polysaccharides (GF-2) isolated from GF-1 is the active component and the small molecules (mw < 10,000 dalton) are excluded, indicating that the ginseng root has no luteo-tropic activities, but the polysaccharides of ginseng flowers have.

Honokiol and magnolol, phenolic compounds isolated from the stem bark of Magnolia officinalis, have been demonstrated to increase choline acetyltransferase activity, inhibit acetylcholinesterase, promote potassium-induced acetylcholine release and exhibit neurotrophic function in in vitro studies. The objective of the present study was to determine the effect of these compounds on hippocampal acetylcholine release in conscious, freely-moving rats. 10^-4 M–10^-6 M of honokiol or magnolol was perfused into rat hippocampus via a dialysis probe. The results showed that at 10^-4 M concentration, honokiol and magnolol markedly increased extracellular acetylcholine release to 165.5 ± 5.78% and 237.83 ± 9.47% of the basal level, respectively. However, lower concentrations of either compounds failed to elicit significant acetylcholine release. This result suggests that a high dose of honokiol or magnolol may enhance in vivo hippocampal acetylcholine release.

The effect of oral administration of 0.25 g/kg/day of Citrullus colocynthis fruits, 0.25 g/kg/day of Rhazya stricta leaves or mixture of the two plants at 0.25 g/kg/day of C. colocynthis fruits plus 0.25 g/kg/day of R. stricta leaves in Najdi sheep was examined. Oral administration of 0.25 g/kg/day of C. colocynthis fruits or 0.25 g/kg/day of R. stricta leaves for 42 days proved not fatal but that of the mixture of the two plants (0.25 g + 0.25 g/kg/day) proved fatal within 26 days with profuse diarrhea, dehydration, loss in condition, ataxia and recumbency, prior to death. These manifestations accompanied by enterohepatonephrotoxicity, gelatinization of the renal and epicardial fat and transudate in serous cavities were correlated with alterations in serum LDH and AST activities and concentrations of total protein, albumin, globulin, bilirubin, cholesterol and urea and hematology.

In the present study, the effectiveness of Sheng-Ji-San (SJS) in promoting the healing of pressure ulcers was evaluated. Thirty-two patients with pressure ulcers were divided into two groups. The treated group received routine medical case combined with SJS treatment and the control group received only routine medical care. After three weeks, the treated group showed a significant reduction in both surface area and depth grade, while the control group showed no significant change. The reduction ratio of the surface area of the ulcer and the effectiveness ratio for the treated group were significantly higher than for the control group. Multiple Linear Regression Analysis revealed that only the SJS treatment was significantly associated with the reduction of surface area and that the use of SJS could independently explain the observed high effectiveness ratio and a relatively high Odds Ratio of 9.539. These results indicate that SJS is effective in promiting the healing of pressure ulcers and should be considered as an adjuvant to routine therapy.

The herbal formula Bu-Zhong-Yi-Qi-Tang (Hochu-ekki-to, 5 grams per day) was administered to five MRSA-infected bedridden patients with cerebrovascular disorder, dementia and, in two cases, bed sores, who had resistance to several antibacterial drugs. After Hochu-ekki-to the patients showed an excellent result; disappearance of MRSA, improvement of their general condition, and no side effects. Therefore, it is thought that Hochu-ekki-to may be a useful drug for MRSA-infected patients.

Nutrition is the major source of bioenergy. The present study investigated the physiological response to fasting by analyzing the effect of fasting on the pulse spectrum of the radial artery. Sixteen subjects were allowed to take only mineral water for 24 hours, and the pulse was measured at intervals during fasting and after eating resumed. Data were analyzed using one-way analysis of variance (ANOVA) with Scheffe's test for pairwise comparisons. The results indicate the second harmonic of the radial pulse increased and the sixth harmonic decreased significantly after 24 hours of fasting. The proportions of the second harmonic then decreased significantly and those of the sixth harmonic increased significantly 12 hours after eating resumed. These findings suggest that a rhythm exists in physiological changes and the distribution of bioenergy, which ensures that the overall heart load will remain unchanged during the fasting and re-feeding periods, maintaining a stable overall balance in body function.

The circadian rhythm of illness has been emphasized for thousands of years in Traditional Chinese Medicine (TCM), using this knowledge to schedule therapies such as acupuncture during appropriate hours of the day. This paper reviews the time periods (two hours segments) that apply to each organ system according to TCM (lungs, large intestine, stomach, heart, kidneys and liver), and elaborates on how recent medical research has found similar relevant biorhythms in these very same organ systems.

Morphological changes of the dermal blood vessels of moxibustion-stimulated rat skin were examined with reference to the lymphocyte migration. After long-term stimulation with direct moxibustion to the acupoint Tsu-San-Li (St-36), peculiar vessels that possess immunohistological features of high endothelial venules could be observed in the moxa-stimulated acupoint dermis. Endothelial cells of the vessels had well-developed Golgi apparatus in their plump cytoplasms, and they strongly expressed intercellular adhesion molecule-l on the luminal surface. These data suggest that the appearance of the peculiar vessels in the dermis acts toward the active infiltration of blood-lymphocytes into the acupoint skin.

In this study, we evaluated whether different frequencies of electroacupuncture (EA) modified the activities of serotonergic neurons in the dorsal raphe (DR) and raphe magnus (RMg) using double labeling immunohistochemistry for Fos and serotonin. The results demonstrated that both high and low frequency EA increased the colocalization between Fos and serotonin in the DR, not in RMg as compared with anesthesia control. In addition, high frequency EA more potently increased the serotonergic activity in the DR rather than low frequency EA, suggesting that serotonergic path-way from the DR plays an important role in the high frequency EA analgesia.

A double blind, randomized, placebo-controlled clinical study was conducted to evaluate the efficacy of laser acupuncture treatment in adolescent smokers. Three hundred and thirty adolescent smokers at the Smoking Cessation Clinic of Child Guidance Clinic, Institute of Health, Singapore, were randomly assigned in equal numbers to laser acupuncture treatment and sham acupuncture (control) groups. The proportions of patients with complete smoking cessation after completing treatment for four weeks were 21.9% in the treatment group and 21.4% in the control group. At three months post-treatment, the rates for complete cessation were 24.8% and 26.2%, respectively. Thus, there was no significant difference in the rates of smoking cessation in the treatment and control groups.
