Background: Phalangeal fractures are common in hand injuries which: comprising of 23% of all hand and forearm fractures. The current consensus is that focus of treatment should be on prompt irrigation and debridement to reduce infection risk. These infections are significant as they can lead to serious sequelae including osteomyelitis. The aim of this study was to determine the incidence of infection amongst patients with open fracture of distal phalanx who had been treated with K-wire fixation and the timing of their operative management compared to the UK national guideline.

Methods: We performed a retrospective case-note analysis of the patients treated for open distal phalangeal fractures at a regional hands centre over the period of 12 months, and compared with the national guidelines. Data collected included patient demographics, mechanism of injury, length of time taken from injury to first washout, length of time K-wire remains in situ, and infection rate.

Results: Half of the patients (n = 19) met the guideline and were treated with washout within the first 24 hours. Infection rates in this group was 11%. This compared with 26% in those patients that did not receive washout within 24 hours.

Conclusions: This study demonstrates the difficulty in always meeting national guidelines and suggests key reasons for this. The authors propose a set of local, easily-achieved interventions to raise awareness and compliance with the national guidelines and reduce infection. Furthermore, it highlights the importance of carefully selecting cases that required percutaneous K-wire fixation.

Background: Carpal tunnel release (CTR) aims to achieve surgical decompression of the median nerve for the treatment of carpal tunnel syndrome (CTS). Flexor tenosynovectomy (FS) has been carried out as an adjunct to routine CTR, on the basis that chronic flexor tenosynovitis has been implicated as an etiological factor in idiopathic CTS. However, the benefits of this additional procedure remains unclear. As such, we aimed to compare functional outcomes, nerve function and complication rates from CTR with and without FS.

Methods: A systematic review of published literature was performed for original data English language studies comparing outcomes of CTR with and without FS in the treatment of primary CTS. Mean weighted differences and their 95% confidence interval were used for analysis.

Results: Three studies comprising 292 wrists were included. Meta-analysis showed no improvement in post-operative grip strength, symptom severity score, functional status score, median nerve motor latency or major complications with FS. Recurrence rate was not reported in the 3 selected articles.

Conclusions: The available evidence suggests FS is an unnecessary adjunct which provides no benefit to CTR, and should not be used routinely to treat primary CTS. Larger studies are needed to validate our findings. FS may have a role in recurrent or secondary CTS.

Background: Volar locking plates have provided the capability to repair both simple and complex fractures. However, complications related to the inability to fix or to maintain the fixation of some fracture patterns have been reported with volar locking plates. The purpose of this study was to evaluate the results of dorsal plating treatment for specific pattern of fractures.

Methods: Patients with distal radius fractures were retrospectively evaluated. Inclusion criteria for this study were those related to the patient and treatment (adult patients, internal fixation with dorsal plating, a minimum follow-up of 12 months), and those related to the fracture pattern (displaced central articular fragment, volar distal fracture line not enough to allow volar fixation, displaced dorsal-ulnar fragment, dorsal partial fractures, combination of these patterns). Clinical outcome information including active range of motion, radiographs, PRWE and DASH questionnaires were collected. Complications were recorded.

Results: During a 6-year period, 679 distal radius fractures were treated with open reduction and internal fixation. Of these, 27 patients fulfilled the inclusion criteria. Patients were examined at a median of 34 months’ follow-up. All but pronation, supination, and radial deviation had a statistically significant difference compared to the opposite side. The median score on the DASH was 4.5 and 3.2 on the PRWE. No patient suffered loss of reduction during the follow-up nor were tendon ruptures recorded.

Conclusions: Although most of the distal radius fractures can be treated with volar locking plates, almost 5% of them present specific patterns that are amenable to treatment with dorsal fixation, without postoperative loss of reduction. These specific patterns are: (1) displaced central articular fragment, (2) volar distal fracture with less of 1cm distance from the distal volar edge of the radius, (3) displaced dorso-ulnar fragment, (4) Barton’s fracture, (5) combination of these patterns.

Background: Previous studies have discussed the diagnostic value of four dimensional kinematic CT in cases of carpal instabilities. This analysis compares calculated skin doses of 4D CT and conventional cineradiography of the wrist in cases of suspected SLL rupture.

Methods: Retrospective calculation and interpolation of skin doses and effective doses for ten consecutive 4D CT examinations and 41 cineradiographies for suspected lesions of the scapholunate ligament. Standardised anterior-posterior and lateral cine sequences using a flat-panel digital subtraction imager and of 4D kinematic CT using a dual-source scanner were acquired and acquisition parameters recorded. We tested if the skin dose of 4D CT is different from cineradiography.

Results: Median dose area product (DAP) of cineradiography was 135.34 cGycm² resulting in a calculated median skin dose of 32.6 mSv (confidence interval 26.86–42.90 mSv) and an estimated effective skin dose of 3.26 µSv. CT dose index (CTDI) for 4D examinations was recorded to be 26.79 mGy and the dose-length product (DLP) was 150 mGy × cm. This resulted in an estimated skin dose of 34 mSv, which is covered by the confidence interval of cineradiography, and an effective skin dose of 3.4 µSv.

Conclusions: Skin dose calculations are comparable for 2D cineradiography in two plains and 4D kinematic CT of the wrist. Calculated effective doses are < 0.01 mSv.

Background: Ganglion cysts are common soft tissue masses of the hand and wrist. It is unclear why some patients pursue surgical excision and others do not. The objective of this study is to compare Patient-Reported Outcomes Measurement Information Systems (PROMIS) scores between patients with ganglion cysts undergoing surgical versus non-surgical treatment.

Methods: Patients presenting to a tertiary, academic hand clinic diagnosed with a wrist ganglion cyst, based on clinical exam, were identified. PROMIS Physical Function (PF), Pain Interference (PI), and Depression scores at the time of diagnosis were reviewed. Data were then stratified according to surgical or non-surgical intervention, with subgroup analysis of dorsal versus volar ganglions and patients that chose to trial cyst aspiration. All results were compared utilizing the appropriate statistical methods.

Results: Of 757 ganglion cyst patients, 264 underwent surgical excision and 493 were treated non-surgically. No difference was noted in PROMIS PF or Depression scores. However, the surgical cohort reported significantly increased preoperative PI scores (Table 2). A higher percentage of patients treated non-surgically received an aspiration attempt compared to those who ultimately received surgery (Table 3). Finally, patients with volar ganglion cysts had significantly increased PI and depression scores, as well as lower PF scores (Table 4).

Conclusions: Presenting PROMIS PI may indicate an increased tendency to pursue surgical treatments and aid in the identification of ganglion cyst patients who are more likely to pursue surgical excision. Further research should focus on correlating absolute differences in PROMIS values with clinical significance.

Background: In the conservative management of distal radial fractures (DRFs), the optimal dorsi-volar angulation of the wrist at cast immobilization and proper cast molding to minimize the risk of redisplacement are unclear. This study identified the predictors of fracture redisplacement during cast immobilization for adult DRFs.

Methods: We examined for dorsi-volar angulation, gap index, volar tilt (VT), radial inclination (RI), and radial length (RL) in lateral or posteroanterior radiographs of 90 DRFs. We investigated possible predisposition factors for redisplacement including patient age, sex, extra- or intra-articular fracture, metaphyseal comminution, original displacements, dorsi-volar angulation of the wrist at cast immobilization, restoration of the volar cortex at cast immobilization, and gap index of the cast.

Results: Neither dorsi-volar angulation nor gap index had significant association with an unacceptable alignment nor decrease of VT, RI, and RL. In multivariate analysis, patient age, original displacement, and intra-articular fracture were the significant predictors of an unacceptable alignment or decrease of VT and RI.

Conclusions: Our findings indicate dorsi-volar angulation and cast molding quality have no clinical effect on preventing fracture redisplacement. The predictive factors of the displacement were patient age, original displacement, and intra-articular fracture.

Background: Children with congenital differences who require prosthesis, have special needs due to their growth and psychosocial factors. Cosmetic or body-powered prosthesis provides basic functional needs but poses a financial burden on the family. Prostheses with advanced functions need to be deferred until adulthood. 3D printed prosthesis is a novel alternative being cost-effective for children. Since limited literature is available on the functionality of 3D printed prosthesis and none in the South Asian population, this study was done to assess its utility in congenital hand amputations.

Methods: Fourteen children with congenital hand amputations were selected for a prospective observational study. Unilateral below Elbow test (UBET), Box and Block test and ABILHAND questionnaire were used for assessment with and without prosthesis after six months.

Results: Eleven patients completed the follow-up. Box and Block test improved from a mean score of 24 to 35 with the prosthesis implying an improvement in manual gross dexterity (p −0.049). UBET (p −0.002) and ABILHAND questionnaire (p < 0.001) showed a decrease in score with the use of a prosthesis which suggested a lack of lateral and tripod pinch in the current design of a prosthesis. Patients with below elbow prosthesis performed better.

Conclusions: 3D printed prostheses are an excellent option for use as a transitional prosthesis as they are inexpensive, serves to improve needs with respect to grasping activities at both wrist and elbow level amputation and customizable according to patients’ need and limb deficiency and replaceable as the child grows with age. Current designs are useful with respect to grasping activities.

Background: The purpose of this retrospective study was to report outcomes of arthroscopic bone grafting and K-wire fixation to treat scaphoid non-union.

Methods: We included in at two healthcare facilities, 42 consecutive patients (34 men, 8 women) with a mean age of 25 years (range 15–56 years) with scaphoid non-union of the proximal and middle third without intracarpal deformity or SNAC osteoarthritis. All patients were evaluated (pain, range of motion, strength, function, X-rays) by an independent examiner.

Results: At the mean follow-up of 18 months (range 12–56), pain was significantly reduced from 7 to 1 on a visual analogue scale (out of 10). The scores on the Quick Disabilities of the Arm, Shoulder and Hand questionnaire and Patient Rated Wrist Evaluation were improved significantly. At the last review, grip strength was 83% of the contralateral side, the average wrist flexion-extension was 125° and the radioulnar deviation was 58°. Bone union was achieved in 37 cases (88%). The mean time of checking the bone union was 3 months (range, 2–8). Four patients required revision surgery because of failed union.

Conclusions: Based on our findings, we found that this procedure can be used as a surgical treatment for scaphoid non-union of the proximal and middle third without intracarpal deformity or osteoarthritis. The arthroscopic bone grafting is a reliable, effective, and minimally invasive procedure.

Background: The intracompartmental septum isolating the extensor pollicis brevis (EPB) has been reported to affect the patient’s response to nonsurgical treatment in de Quervain’s disease. A simple physical test called the modified staged Finkelstein test was developed to evaluate the septum; the result of this test was compared with those of the pre-existing physical test (EPB entrapment test) and ultrasonographic (US) examination of the septum.

Methods: We retrospectively analyzed 52 patients who underwent two clinical tests, including the modified staged Finkelstein test and the EPB entrapment test, and US examination for de Quervain’s disease. The correlation between the clinical test results and US findings was evaluated; sensitivity, specificity, and positive and negative predictive values were calculated.

Results: The proportion of wrists with a separate septum was 50% (26 patients) in the US examination. The sensitivity and specificity of the modified staged Finkelstein test were 88.5% and 73.1%, respectively; those of EPB entrapment test were 71.4% and 84.2%, respectively. The positive and negative predictive values of the modified staged Finkelstein test were 76.7% and 86.4%, respectively.

Conclusions: The modified staged Finkelstein test showed acceptable diagnostic values for the diagnosis of septum compared with pre-existing physical tests. Knowledge about the existence of septum could be helpful in treating patients and expecting prognosis.

Background: Olecranon fractures are common in motorcycle accidents, falls, or in direct elbow injury. In cases of transverse olecranon fracture, surgery is frequently required in adults. The aim of this study is to compare the biomechanical durability of suture anchor fixation in olecranon fractures to conventional tension band wiring technique in saw bones.

Methods: 12 plastic saw bones were divided into 3 groups: tension band wiring fixation, modified Cha-Batman method, and a modified simple suture method using a suture anchor. After fixation, cyclic load tests were conducted for 1,000 cycles, at 5 Hz with a force of 10 N to 250 N. After cycling loading, the extent of displacement was measured using a non-contact coordinate measuring instrument, and statistical analysis performed.

Results: The average displacement was significantly smaller in the modified Cha-Bateman method (1.4 mm) than in the tension band wiring method (3.8 mm, p = 0.007) and the modified simple suture method using suture anchor (3.3 mm, p = 0.012). There was no significant difference in displacement between tension band wiring fixation and the modified simple suture method (p = 0.564).

Conclusions: This study provides a biomechanical basis for the hypothesis that the suture anchor technique in weak bone model could obtain results comparable to those of conventional tension band wiring. In particular, the modified Cha-Bateman method, showed stronger biomechanical properties than the tension band wiring method and modified simple suture method using a suture anchor. The current study could also provide pilot data that can be used in future experiments.

Background: The trans-olecranon approach is commonly used to treat intra-articular distal humeral fractures (DHFs). We describe an osteotomy site repair technique that is both simple and safe.

Methods: We retrospectively reviewed 30 patients with intra-articular DHFs who were treated via olecranon osteotomies repaired by tension band wiring (TBW) with ring pins. Medical records and radiographs were retrospectively assessed in terms of injuries, operative characteristics, clinical outcomes, and complications, as well as any need for hardware removal. Clinical outcomes were evaluated by deriving the range-of-motion and the Mayo elbow performance score.

Results: Thirty patients (nine men, 21 women; mean age, 49.7 years) with Arbeitsgemeinschaft für Osteosynthesefragen (AO) type 13-B and 13-C DHFs were included; the mean follow-up time was 49.9 months (range, 12–145 months). Anatomical reduction and bony union were achieved at all osteotomy sites. The mean elbow flexion was 121.7° (range, 100–135°) and the mean elbow extension was 11.3° (range, 0–30°). The mean Mayo elbow performance score was 90 points (range, 55–100 points); outcomes were excellent in 15 patients, good in 12 patients, fair in two patients, and poor in one patient. Olecranon implant removal was performed for 10 patients (33.3%; total removal in nine and isolated olecranon implant removal in one). Four of these patients (13.3%) complained of olecranon implant discomfort. Implants were removed during other surgical procedures from the remaining six patients (20%). No implant migration/breakage or wound complications were encountered.

Conclusions: TBW with ring pins is a simple and safe method for olecranon osteotomy site repair.

Background: Flexor tendon injuries form a significant proportion of hand trauma presentations. Insult to the flexor tendon and surrounding sheath may lead to the formation of adhesions between these structures during the repair process. Tenolysis is a surgical release of these adhesions which requires careful consideration. This systematic review aims to report on the functional outcomes following isolated digital flexor tenolysis.

Methods: We searched four online databases in December 2019. Eligibility criteria for studies were: English language; described patients undergoing digital flexor tenolysis; reported functional outcomes such as total active motion (TAM). Five articles were included. The selected studies were of limited quality (level IV evidence).

Results: Of the final cohort 79.4% were male. Three studies reported patient age, with a mean age of 36.8 years [4 years to 58 years] in that subset. One hundred and three digits underwent flexor tenolysis only. Mean duration to surgery from the index operation or injury was 15.1 months [2.3 months–240 months]. Average follow-up from the tenolysis procedure was 18.6 months [3 months to 120 months]. Four studies assessing motion via Strickland classification reported 78.9% excellent or good outcomes. The remaining study reported 80% of patients reporting excellent or good motion as per Buck-Gramcko score. The complication rate reported was 15.3%.

Conclusions: This review found digital flexor tenolysis significantly improves ROM, however these benefits must be considered in the light of potential complications. A detailed discussion with each individual patient must occur prior to such an undertaking.

Background: Spaghetti wrist injuries involve a complete transection of three or more structures at the wrist level (including tendons, nerves, or arteries). Few studies on spaghetti wrist trauma have been reported in the literature, and most of them have focused on functional recovery rather than the prognostic factors which affect the outcome. We attempted to address some of the lacunae in current knowledge.

Methods: Patients who were operated between January 2017 to January 2019 for spaghetti wrist injuries at a level 1 trauma center were included in the study. Several variables such as age, gender, smoking, education level, mechanism of injury, time gap between the injury to surgery, number of damaged structures and pattern of neurovascular structure were selected as potential prognostic factors to be included in the analysis. Motor recovery, sensory recovery & QuickDash score were used for the outcome assessment. A Multivariate analysis was done to identify the prognostic factor(s).

Results: Thirty patients were included in the study. The mean age was 30.5 years (range: 7–57 years). Male population was predominantly affected (93.33%). The dominant limb was injured in 73.33%. The most common mechanism of injury was accidental glass cut (n = 19,63.33%), the most frequently affected structure was flexor digitorum superficialis of the middle finger (n = 23, 76.6%). Combined nerve injury was present in 23.33%. Intrinsic muscle recovery was affected significantly by presence of crush injury (e = 7.189, std error = 2.425, p = 0.003), education significantly affected power grip recovery (p < 0.0001), age was associated with pinch grip recovery (e = 0.083, std error = 0.039, p = 0.034).

Conclusions: Increasing age, low education level, and presence of crush injury were identified as negative prognostic factors in the study. These findings may be used for counselling of patients affected by spaghetti wrist injuries.

Background: Collagenase Clostridium histolyticum (collagenase) is an injectable treatment option for Dupuytren disease. The current study was designed to investigate the safety and ensure the effectiveness of collagenase for the treatment of Dupuytren disease, with or without concomitant antithrombotic usage.

Methods: One hundred and forty-eight patients with Dupuytren disease were treated with collagenase during this period; 49 taking antithrombotics and 99 not taking antithrombotics. The primary outcomes were clinical success (a reduction in joint contracture to < 5°) and clinical improvement (a reduction in joint contracture by equal to or more than 50%).

Results: No statistically significant difference in either clinical success or clinical improvement was found between those taking and those not taking antithrombotics. No significant difference was found in the incidence of any adverse effects or skin splits between the two cohorts.

Conclusions: Collagenase can be safely and effectively used to treat patients with Dupuytren disease who take antithrombotics.

Background: The asymmetric technique for flexor tendon repairs has various advantages but further improvement is possible by using a suitable suture material. In this study, we compare the biomechanical performance of asymmetric repairs using 3 suture materials: Supramid Extra II 4-0, Tendo-Loop 4-0, and FiberLoop 4-0.

Methods: We divided 30 porcine flexor tendons into 3 arms. Each arm of tendons were repaired by using 1 of the 3 proposed suture materials. Each repaired tendon was tested by using a mechanical tester. Ultimate tensile strength (UTS), load to 2 mm gap force, failure mechanism and stiffness of the tendons were investigated.

Results: FiberLoop® achieved the highest UTS (90.4 ± 17.9 N), followed by Tendo-Loop (85.3 ± 10.3 N). The difference was no statistically significant. The UTS of repairs using Supramid® was significantly lower (64.0 ± 8.4 N). For load to 2 mm gap force, FiberLoop® achieved 20.7 ± 4.8 N while Tendo-Loop® had 20.5 ± 4.2 N. The difference was also no statistically significant. The repairs using Supramid® had a significantly lower load to 2 mm gap force of 14.2 ± 2.7 N. The stiffness of asymmetric repairs using FiberLoop® and Tendo-Loop® was 5.64 ± 1.7 N/mm and 5.63 ± 0.7 N/mm. The difference was also no statistically significant. The repairs using Supramid® had a significantly lower stiffness of 4.15 ± 1.0 N/mm. Failure mechanisms for the repaired specimens were reported as such: Supramid had 100% suture rupture; Tendo-Loop had 90% suture rupture and 10% suture pullout; FiberLoop had 20% suture rupture and 80% suture pullout.

Conclusions: The asymmetric repair configuration is able to produce superior biomechanical performance by using Tendo-Loop® or FiberLoop®. Improvement of the asymmetric technique by using Tendo-Loop® or FiberLoop® could potentially contribute to better surgical outcomes of flexor tendon repair.

Background: Limited research exists investigating the association between diabetes and glycemic control on complications following elective hand surgery. The goal of this research was to assess the incidence of complications within 30 days of elective hand surgery in a large population of diabetic patients compared to a population of non-diabetics. Furthermore, we sought to examine the relationship of glycemic control, as measured by HbA1c, and postoperative complications.

Methods: We performed a retrospective review of electronic medical records at our institution of all patients who underwent elective hand, forearm, or elbow surgery from the dates of January 1^st, 2008 to December 31^st, 2017. Patients were categorized as diabetic or non-diabetic and most recent HbA1c was documented. Multivariable analysis was employed to compare the incidence of surgical complications within 30 days between the diabetics and non-diabetic populations, adjusting for baseline patient characteristics.

Results: A total of 3,261 patients met the inclusion criteria. There were 646 (20%) diabetic patients and 2,615 (80%) non-diabetic patients. No difference was found in the overall rate of complications between the cohort of diabetic and non-diabetic patients. Additionally, statistical analysis found no difference in the complication rate between insulin and non-insulin controlled diabetics. Rates of complications were stratified based on HbA1c level and statistical analysis found no increased risk of complications with increased hemoglobin A1c value.

Conclusions: In our present study we were not able to demonstrate any significant difference in the 30 day complication rates between and non-diabetics undergoing elective hand surgery. This study attempted to aid in risk stratification of diabetic patients by evaluating preoperative glycemic indices in the form of HbA1c.

Background: Proximal Row Carpectomy (PRC) is a widespread, safe and effective salvage surgical procedure for wrist arthritis. Some authors believe that PRC results in low grip strength (GS), due to the loss of carpal height, supporting the idea to discourage PRC in high-demanding patients. Resurfacing Capitate Prosthesis Implant (RCPI) allows extending the indication for PRC also in case of deformity and/or arthritis of the head of capitate, with possible implications of clinical outcomes, including GS.

Methods: Retrospective multicentre study on a population of active workers, affected by secondary post traumatic wrist arthritis, who underwent PRC (27 patients) or PRC + RCPI (20 patients), Primary outcome was to assess GS between PRC and PRC + RCPI. Secondary outcome was to assess CHR and to search for any possible contributors to GS. Active range of motion (AROM), hand function (DASH, Work-DASH, VAS, PRWHE), pain, time to return to work, job maintenance, major complications and general satisfaction were also assessed.

Results: PRC + RCPI results in more GS maintenance compared with PRC alone, with higher values of CHR. CHR values were associated with GS with a good correlation. According to linear regression model analysis within PRC + RCPI group (GS–CHR), it is esteemed that the increase in parameter CHR is associated with an increase in parameter GS. Looking at a multiple linear regression model analysis built on the whole sample (GS% increase – (group × CHR) + GS% pre-operative). It is estimated that the increase of one unit of the GS coefficient is associated with an increase in GS% increase. Furthermore, higher pre-operative GS values positively influence post-operative GS. No differences were revealed between the two treatments in terms of the remaining secondary outcomes.

Conclusions: PRC alone and PRC + RCPI are both effective salvage procedures for wrist arthritis. RCPI provides a better GS preservation, in part due to the carpal height preservation.

Background: The precise etiology of carpal tunnel syndrome (CTS) remains unclear. One of the accepted factors for CTS is the restriction of the median nerve. Previous reports using ultrasound had only observed and measured the movement of parts of the median nerve. In this study, we aimed to elucidate the difference in the movement of the entire median nerve in patients with CTS (before and after surgery) and healthy volunteers using a new measurement method.

Methods: We expressed the amount of movement of the entire nerve by a new method creating the motion area of the median nerve (MAMn) from an ultrasonographic video image on the computer. We compared the MAMn, the real MAMn (RMMn) (the value obtained by subtracting the nerve cross-sectional area from the MAMn), and mobile ratio (MR) (the value obtained from dividing the MAMn by the nerve cross-sectional area) between six wrists of six cases of CTS (before and at an average of 3.5 months after surgery) and six wrists of six healthy volunteers.

Results: During passive wrist flexion, the average MAMn, RMMn, and MR of healthy cases were 23.1 mm², 16.4 mm², and 3.52, respectively. The average MAMn, RMMn, and MR of cases of CTS were respectively 11.8 mm², 5.4 mm², and 1.86 preoperatively; and 16.2 mm², 7.3 mm², and 1.87, postoperatively. The MAMn, RMMn, and MR decreased more significantly in patients with CTS than in healthy volunteers (p < 0.01). The MAMn and RMMn increased postoperatively (p < 0.05), but the MR remained low.

Conclusions: The new measurement method revealed that the mobility of the entire median nerve was significantly restricted in cases of CTS compared to healthy participants. However, after surgery, nerve restriction was not restored despite improvements in symptoms, suggesting that decreases in nerve mobility contribute to CTS but are not a direct cause of symptoms.

Background: No consensus exists regarding the management of complete collateral ligament injuries of the proximal interphalangeal joint (PIPJ) of fingers.

Methods: We aimed to systematically review the outcomes of Acute (< 1 month) surgical repairs of these injuries. Outcomes assessed included Stability, Pain, Range-of-Motion and Return to Function. The Modified Coleman Methodology Score (MCMS) was utilised in critical appraisal.

Results: 70 patients with complete collateral ligament injuries of the PIPJ were identified in 5 studies. 49 were managed operatively and 21 non-operatively. All trials were methodologically flawed with a mean MCMS of 50.4, corresponding to a “Poor” Level of evidence.

Conclusions: Whilst acute surgical repair of complete collateral ligament injuries of finger PIPJs are a described viable management option with promising results, there is insufficient high-quality evidence to inform current practice. Based on the current literature, no evidence-based conclusions can be made regarding superiority of acute surgical repair over conservative management or one method of surgical repair over another. Further high level studies are required.

Background: Pneumatic tourniquet is an effective tool to achieve hemostatic control of the surgical field in upper extremity (UE) operations. Elevated pressures have been associated with adverse effects despite various methods of pressure determination. We aim to demonstrate the usage of reduced tourniquet pressures and examine factors associated with achieving reduced pressures.

Methods: A prospective study was conducted (2016–2018) at a Level 1 Trauma Center and an Outpatient Surgical Center, totaling 226 operations, involving a reduction of cuff pressures over time from a standard baseline of limb occlusion pressure for UE operations.

Results: A gradual reduction of pressures was successfully achieved with a mean pressure of 187 mmHg and average time of tourniquet application being 25 minutes. We found chronological surgical number and patient BMI to be significantly associated with tourniquet pressure (p < 0.05). 4.5% of cases resulted in breakthrough bleeding, but did not reliably occur with any pressure thresholds, patient demographics, or operative factors (p > 0.05, for all).

Conclusions: Reduced tourniquet pressures can mitigate complications associated with tourniquet use. Our research shows reduced pressures are successful in maintaining field visibility and we encourage an adoption of pressures below 200 mm Hg in most procedures that require a tourniquet.

Background: Flexor tendon rehabilitation protocols minimize repair tension by limiting range of movement to prevent tendon rupture. The resultant muscle contracture inhibits finger extension, increases resistance in tendon gliding distally, and progress to proximal interphalangeal (PIP) joint flexion contracture. This study describes our new rehabilitation protocol, the Tension Reducing Muscle Stretch (TRMS), designed to prevent flexor muscle contracture and obtain full distal tendon excursion.

Methods: We reviewed retrospectively 14 fingers in 13 consecutive patients with primary repair of complete zone I or II flexor digitorum profundus (FDP) tendon rupture were treated with our protocol between 2007 and 2019. Our rehabilitation following FDP 4-strand repairs consisted of three steps. The first step comprised of exercises from traditional protocols such as Duran, Kleinert, Synergistic-wrist-motion, and Place-and-hold. The second step comprised the TRMS exercise to prevent the onset of muscle contracture. Anatomically, FDP tendons arise from the same FDP muscle belly. TRMS involved placing the affected finger in full passive flexion while unaffected fingers were passively extended to full extension. This made the affected FDP muscle stretched. The final step incorporated the early active flexion motion exercise, in which simple fisting was performed, from a fully extended position.

Results: The mean total active motion at the final follow up was 235° (range 170–265). Using the Strickland criteria, eight achieved excellent, four had good, two had fair results. The mean angle of passive extension deficit at the PIP joint at four weeks after surgery was −7° (−30–0), and at the final follow up was −3° (−20–0). No tendon repair was ruptured.

Conclusions: This protocol reduced tension in the affected tendon muscle and encouraged tendon excursion distal to the repair site without complications. It allows full tendon excursion and prevents PIP joint contractures.

Background: Patient-reported outcome measures are largely used in clinical practice and scientific community. Michigan hand questionnaire (MHQ) is widely recognized as a standardized reliable instrument for the assessment of any kind of hand disorders. Aim of the study: translation, cross-cultural adaptation and validation of the Italian version of MHQ.

Methods: The study was composed by two phases. Phase 1 consisted in translation and cross-cultural adaptation of MHQ, from original language version (English) into Italian, according to the standard procedure of translation and back translation. The final Italian version of MHQ was tested on 136 Italian patients with hand disorders (Phase 2), in addition to Disabilities of the Arm, Shoulder, and Hand (DASH) questionnaire, Visual analogue scale (VAS) for pain assessment and grip strength, to psychometric properties of the new version, 55 patients repeated the questionnaire after 7 days for test-retest to assess the reproducibility of the questionnaire. All data were subsequently analyzed (descriptive statistics, multitrait analysis, reliability and construct validity assessment).

Results: Phase 1 was performed without major problems, thus the final Italian version was approved for Phase 2. The questionnaire was clear and easily understood (missing data 0–2.9%). Multitrait analysis brought very good results for each outcome measure. High reliability (Cronbach’s alpha: 0.91–0.99) and very good reproducibility (Intraclass correlation coefficients: 0.83–0.98) were revealed. High to moderate correlation was found between MHQ and DASH, grip strength and VAS.

Conclusions: The Italian version of MHQ has demonstrated to be reliable and valid.

Background: When nonunion of scaphoid waist is treated with non-vascularized bone grafting with internal fixation, bony union would be affected not only type of bone graft but also fixation method. Thus, we attempted to investigate whether the bony union depends on how surgeons combine bone grafting and internal fixation.

Methods: We treated 38 consecutive patients with unstable nonunion of scaphoid waist. The patients were treated with one of three types of non-vascularized bone grafting and internal fixation by random sampling, regardless of the configuration of the nonunion; cortico-cancellous bone grafting with Kirschner wire (K-wire) fixation group (12 cases), cortico-cancellous bone grafting with headless compression screw fixation group (13 cases) and cancellous bone grafting with K-wire fixation group (13 cases). We compared “union rate” and “time to union” between groups with statistical analysis. We described clinical features of “failure to union” cases.

Results: The mean time to union of cancellous bone grafting with K-wire fixation group was significantly shorter than those of other groups. There was also significant difference in mean time to union between cortico-cancellous bone grafting with K-wire fixation group and cancellous bone grafting with K-wire fixation group (when the type of bone grafting was different). All the “failure to union” cases were sclerotic (Herbert type D2) nonunion treated by cortico-cancellous bone grafting.

Conclusions: When treating unstable nonunion of scaphoid waist with non-vascularized bone grafting with internal fixation, cancellous bone grafting with K-wire fixation seems to be advantageous in terms of mean time to union. It seems that both bone grafting and fixation method affected “mean time to union” but the type of bone grafting was more influential in achieving union. The failure to union might be frequent in case of Herbert type D2 nonunion combined with cortico-cancellous bone grafting.

Background: A major consequence of the COVID-19 pandemic on the U.S. healthcare system has been the rapid transition away from in-person healthcare visits to telehealth. This study analyzed patient and surgeon satisfaction in the utilization of telehealth within the hand surgery division during the COVID-19 pandemic.

Methods: All hand surgery patients who completed a telemedicine visit from March 30^th, 2020 through April 30^th, 2020 completed a 14-question survey via e-mail. Hand surgeons who participated in telemedicine completed a separate 14-question survey. Survey results were presented descriptively (mean ± standard deviation) and patient factors influencing satisfaction were determined using univariate and multivariate proportional modeling.

Results: 89 patients and five surgeons completed the surveys. Patients were very satisfied with their telemedicine visits (4.21/5.00 ± 0.89). Multivariate proportional modeling determined patients who found it “very easy” (5/5) to arrange telemedicine visits had greater satisfaction (OR = 4.928; 95% CI = 0.94 to 25.84) compared to those who found it “difficult” (2/5) (p = 0.059). Patients who believed they could ask/relay questions/concerns “extremely effectively” (5/5) had greater satisfaction (OR = 55.236; CI = 11.39 to 267.80) compared to those who asked/relayed questions only “slightly effective” to “moderately effectively” (p < 0.001). Surgeons were similarly satisfied with their telemedicine experience (4.00/5.00 ± 0.89) and were confident in their diagnoses (4.20/5.00 ± 0.84). All surgeons responded they will continue using telemedicine. 30.7% of patients would choose telemedicine over an inperson visit.

Conclusions: Telemedicine provides a viable platform for healthcare delivery with high patient and surgeon satisfaction. Most patients still prefer in-person visits for the post-pandemic future.

Volar plate fixation (VPF) of scaphoid fractures has received increased attention over the past decade. The purported benefits over headless screw fixation are increased rigidity, better purchase of small fragments, the ability to prevent extrusion of bone graft, and to act as a buttress against excessive scaphoid flexion. We report a case of symptomatic radioscaphoid impingement presenting two years after successful VPF for a non-united scaphoid fracture. We performed an arthoscopic evaluation, synovectomy and chondroplasty, followed by open radial styloidectomy and implant removal. This article highlights the pertinent clinical features, relevant imaging, and key intra-operative findings. We have analyzed the factors that led to this complication and have highlighted several technical tips to minimize radiocarpal impingement and ongoing chondral damage.

Aneurysmal bone cyst (ABC) is a benign expansile bone tumor without metastasis capability. Only 3–4% of ABCs occur in the hand and they mainly take place in metaphysis’ of long bones like metacarpals or phalanges. Carpal ABCs have been reported as individual case reports in the literature due to rarity. A patient presented with pain in her right wrist. Magnetic resonance imaging revealed a well circumscribed one cm sized mass in the pisiform bone that resembled an aneurysmal bone cyst. Total pisiformectomy was performed. Treatment options are total excision or curettaging in ABCs. But rarity of these lesions may delay the diagnosis process for the inexperienced surgeon.

Thumb carpometacarpal joint osteoarthritis is a common condition that is treated nonsurgically. When conservative treatments fail, surgery is considered. Recently, suture button suspensionplasty has attracted attention and yielded good clinical results. We have developed a novel suture button suspensionplasty procedure called “cross-coupling suture button suspensionplasty” (CC-SBS) using a pair of suture button devices after a complete trapeziectomy. This retrospective study aimed to report the short-term clinical results of patients who underwent CC-SBS at our institution. We evaluated 10 hands of 10 patients (9 women and 1 man; average age, 67 years) who were refractory to conservative treatment and underwent CC-SBS at our institution between 2015 and 2017. We evaluated the preoperative Eaton stage and postoperative trapezial space height immediately after surgery and at the final visit; measured the trapezial space height, range of motion of the palmar and radial abduction of the carpometacarpal joint at the final visit; measured the preoperative and postoperative pinch strength; and recorded postoperative complications. The mean trapezial space height was 6.5 mm, and the mean radial and palmar abductions were 47° and 45°, respectively, at the final visit. The average pinch strength improved to 3.8 kg at the final visit compared to that pre-operation. In one case, a second metacarpal fracture occurred 2 weeks post-surgery, but no other complications were reported. Conclusion: CC-SBS showed short-term clinical outcomes similar to those of ligament reconstruction and tendon interposition and demonstrated faster overall recovery. Our procedure does not need a donor tendon for suspension and is technically simple and less invasive than ligament reconstruction tendon interposition. Thus, stronger initial fixation is obtained by using two suture button devices, and rehabilitation can be started from an early stage. We believe that this procedure is a good surgical option for carpometacarpal joint osteoarthritis.

Avulsions of the flexor digitorum profundus (FDP) tendon and phalanx fractures are both common injuries for rugby players, but these concurrent injuries in the same finger have not been previously described. This case describes a 20-year-old female rugby player who sustained a right ring finger comminuted, mildly displaced middle phalanx fracture. The patient was evaluated by a hand surgeon 3 weeks after the injury, and non-operative management with a splint was elected. One week later, upon further examination, the patient demonstrated persistent inability to flex the distal interphalangeal joint (DIPJ) of the ring finger. Ultrasound was inconclusive but an MRI demonstrated avulsion of the FDP tendon from the distal phalanx, consistent with a jersey finger injury. The patient subsequently underwent open repair of the FDP tendon. This case illustrates the importance of careful physical exam and index of suspicion for coexisting injuries.

Hemosiderotic synovitis is a rare distinctive type of synovial proliferative disorder. Hemosiderotic synovitis develops in men with hemophilia who are at risk of hemarthrosis, especially of the knees, associated with severe degenerative articular cartilage disease. It appears to be exceedingly rare in individuals without hemophilia and has been reported in case reports. We describe a case of trauma-related nonhemophilic hemosiderotic synovitis, located in the hand in an otherwise healthy young male construction worker, with no systemic illness, inflammatory arthritis, or any bleeding disorder. We report the first case of nonhemophilic hemosiderotic synovitis located in the hand. To the best of our knowledge, a case of extra-articular nonhemophilic hemosiderotic synovitis has not been previously described in the literature.

Replantation of multilevel amputation of the hand requires considerable hospital resources, and the surgical outcomes in older adults have not been described in detail. Thus, replantation for this injury was mostly confined to young patients. Here, we describe the case of a 63-year-old patient with multilevel amputation of the hand in whom replantation surgery was successful with grasp and pinch functions by the last follow-up. We report the clinical, functional, and patient-reported outcomes and discuss the indications. As the patient transfer system and communication technology develops, more patients will arrive at hospitals in a critical time for replantation. Accordingly, hand surgeons should consider offering replantation option for multilevel amputation after evaluating the indications.
