    Study on Joint Injury of Taijiquan Movement Based on Computer Image Analysis

    As a traditional Chinese martial art, Taijiquan has a remarkable effect on the rehabilitation of joint injury with its unique movement. In this paper, the influence of Taijiquan on joint injury is analyzed by using the local depth feature representation method of image sampling. Then, the local feature coding algorithm is introduced, and the problems existing in the rehabilitation of joint injury are analyzed. An analysis algorithm of the influence of Taijiquan on joint injury based on CV model was proposed, and the effectiveness of the algorithm was verified. The results show that the proposed algorithm improves the MS-COCO dataset by 0.2%, 0.88%, 1.86% and 3.18%, respectively, compared with Hash Net. On the 15Scene dataset, CNN-VLAD’s classification results were 4.1% higher than those of the TNNCV model. On the Caltech 256 data set, the classification accuracy of SMVLADC algorithm is 7.7% higher than CNN-VLAD algorithm. This shows that the proposed algorithm is effective, and the local depth features extracted by CNN are more effective than the traditional artificial features. At the same time, the superiority of CV model based on improved significant regional features is further verified. This study provides a new theoretical basis and practical method for the rehabilitation treatment of joint injury by Taijiquan.

    Effects of Extended Tai Chi Intervention on Balance and Selected Motor Functions of the Elderly

    Balance ability decreases with age, which results in an increased risk of falls for people over age 65. Tai Chi exercise appears to offer potential benefits in the reduction of falls for the elderly. The purpose of the present study was to examine the effects of extended (6- and 12-month) Tai Chi exercise interventions on balance and selected motor functions for senior citizens. Forty-seven subjects were recruited from two local senior centers. Twenty of them (M = 71.8 years, SD = 7.1), 11 in the Tai Chi exercise group and 9 in the control group, completed the pre-, mid- and post-tests over 12 months on five selected functional performance tests: static balance, dynamic balance, choice reaction time, heel-rise strength, and ankle flexibility. The Tai Chi group was provided with a one-hour Tai Chi exercise session per week for 12 months; the control group did not participate in any exercise program. Results showed that static balance improved significantly after a 6-month Tai Chi intervention. Moreover, the Tai Chi group maintained a higher level in the test performance compared with the control group at the end of the 12-month intervention, but there was no significant difference between the two groups. Data suggested that Tai Chi exercise intervention could produce a positive influence on balance control for the elderly over a prolonged period, but not on muscle strength and ankle flexibility.

    Effects of a Taiji and Qigong Intervention on the Antibody Response to Influenza Vaccine in Older Adults

    Previous studies have suggested that Taiji practice may improve immune function. This study was intended to examine whether 5 months of moderate Taiji and Qigong (TQ) practice could improve the immune response to influenza vaccine in older adults. Fifty older adults (mean age 77.2 ± 1.3 years) participated in this study (TQ N = 27; wait-list control [CON] N = 23). Baseline pre-vaccine blood samples were collected. All subjects then received the 2003–2004 influenza vaccine during the first week of the intervention. Post-vaccine blood samples were collected 3, 6 and 20 weeks post-intervention for analysis of anti-influenza hemagglutination inhibition (HI) titers. We found a significant (p < 0.05) increase in the magnitude and duration of the antibody response to influenza vaccine in TQ participants when compared to CON. The vaccination resulted in a 173, 130, and 109% increase in HI titer at 3, 6, and 20 weeks post-vaccine, respectively, in the TQ group compared to 58, 54, and 10% in CON. There was a significant between group difference at 3 and 20 weeks post-vaccine and at 20 weeks the TQ group had significantly higher titers compared to the pre-vaccine time point, whereas the CON group did not. A higher percentage of TQ subjects also responded to the influenza A strains with a protective (> 40HI) antibody response (37% TQ vs. 20% CON for the H1N1 strain and 56% TQ vs. 45% CON for the H3N2 strain), but the differences between groups were not statistically significant. Traditional TQ practice improves the antibody response to influenza vaccine in older adults, but further study is needed to determine whether the enhanced response is sufficient to provide definitive protection from influenza infection.

    Effects of Tai Chi on Gait Kinematics, Physical Function, and Pain in Elderly with Knee Osteoarthritis — A Pilot Study

    Our previous study has demonstrated that 6 weeks of Tai Chi exercise significantly improves knee pain and stiffness in elderly with knee osteoarthritis. This study also examine the effects of Tai Chi exercise on gait kinematics, physical function, pain, and pain self-efficacy in elderly with knee osteoarthritis. In this prospective, pretest-posttest clinical trial, 40 men and women (64.4 ± 8.3 years) diagnosed with knee osteoarthritis participated in 6 weeks of instructed Tai Chi training, 1 hour/session, 2 sessions/week. The following measures were taken at baseline and the conclusion of the intervention: (a) gait kinematics including stride length, stride frequency, and gait speed quantified using video analysis, (b) physical function, (c) knee pain, and (d) pain self-efficacy. Data were analyzed using repeated MANCOVA, MANOVA, ANOVA and Wilcoxon tests. After 6 weeks of Tai Chi exercise, stride length (p = 0.023; 1.17 ± 0.17 vs. 1.20 ± 0.14 m), stride frequency (p = 0.014; 0.91 ± 0.08 vs. 0.93 ± 0.08 strides/s), and consequently gait speed (p < 0.025; 1.06 ± 0.19 vs. 1.12 ± 0.15 m/s) increased in the participants. Physical function was significantly improved (p < 0.001) and knee pain was significantly decreased (p = 0.002), while no change was observed in pain self-efficacy. In conclusion, these findings support that Tai Chi is beneficial for gait kinematics in elderly with knee osteoarthritis, and a longer term application is needed to substantiate the effect of Tai Chi as an alternative exercise in management of knee osteoarthritis.

    The Effects of Tai Chi on Body Balance in Elderly People — A Review of Studies from the Early 21st Century

    We performed a systematic review of studies regarding the effects of Tai Chi on the body balance of people older than 60 years. The Medline, SPORTDiscus and Academic Search Complete databases were searched for relevant studies published after the year 2000. Original randomized controlled trials, non-randomized controlled studies and observational studies were included in this review if they evaluated Tai Chi for the treatment of body balance disorder or fall prevention, and if they contained at least one of the following key words: Tai Chi, body balance, stability, elderly, old, training or posturography. These studies suggest that participation in Tai Chi may improve the body balance of elderly people; however, more information is needed to determine the extent to which Tai Chi is more effective than other methods, especially different types of physical exercise. In addition, subgroup analyses should be performed to determine the effects of Tai Chi in people with different physical characteristics. Directions for future research are discussed.

    Long Term Tai Chi Exercise Improves Physical Performance Among People with Peripheral Neuropathy

    This study examined the effects of a 24-week Tai Chi intervention on physical function in individuals with peripheral neuropathy. Twenty-five women and men with peripheral neuropathy were recruited. Plantar pressure detection threshold was assessed with a 5.07 gauge monofilament. Functional gait was assessed by the 6-min walk and timed up-and-go tests. Isokinetic leg strength and standing balance was also assessed. Twenty-four consecutive weeks of modified, group-based Tai Chi practice was completed, with testing repeated every six weeks throughout. No adverse events were observed and attendance was 17 ± 4 sessions per 6 weeks. After 6 weeks of Tai Chi, participants increased 6-min walk (P < 0.0001), timed up-and-go (P < 0.0001), and leg strength (P < 0.01) performance. Continued improvement was observed in the timed up-and-go. Plantar sensation improved (P = 0.003) following the Tai Chi intervention. Group-based Tai Chi is a safe, plausible, and effective intervention for those with PN.

    Effects of Tai Chi Rehabilitation on Heart Rate Responses in Patients with Coronary Artery Disease

    The objective of the present study was to evaluate the effect of a six-month Tai Chi (TC) exercise cardiac rehabilitation program on two prognostic factors of cardiac events, rate-pressure product and rate-pressure product reserve, in patients with coronary artery disease (CAD). Patients (N = 54) with CAD were recruited from the clinics of cardiology and cardiovascular surgery at a regional hospital in Taiwan. Twenty-two of them enrolled in the TC rehabilitation program which consisted of weekly 90-min sessions of Yang's style TC for six months in addition to receiving usual care. The remaining 32 patients received usual care only. Modified Bruce treadmill exercise test was performed to evaluate their exercise test responses at baseline and at six months. The change over time was significantly different between the TC and control group in peak rate-pressure product (RPP) (interaction between group and time, p = 0.029) and in RPP reserve (interaction between group and time p = 0.009) over the six-month period, there was a decrease in peak RPP of 32.0 mmHg × bpm × 10-2 and in RPP reserve of 37.4 mmHg × bpm × 10-2 in the TC group. In conclusion, participating in a six-month TC exercise-based cardiac rehabilitation program was associated with improved peak RPP and RPP reserve during exercise testing in patients with CAD. TC exercise program may lead to a better prognosis for cardiac events in patients with CAD.

    Effects of Long-Term Tai Chi Practice on Balance and H-Reflex Characteristics

    The purpose of the present study was to examine the effects of long-term Tai Chi practice on postural balance and H-reflex. Sixteen healthy volunteers, eight with three or more years of experience in Tai Chi training (Tai Chi Group-TCG), and eight with no experience in Tai Chi training (Control Group-CG) participated in the study. Postural sways were measured under four experimental conditions: (1) Standing still with eyes open (EO); (2) Standing still with eyes closed (EC); (3) Standing and turning head to left and right with eyes open (EOT); and (4) Standing and turning head to left and right with eyes closed (ECT). Paired reflex depression (PRD) of the soleus muscle was measured under two conditions: supine and standing. Less significant postural sway was observed in the TCG than in the CG under four conditions including EO, EC, EOT, and ECT (p < 0.01). The TCG demonstrated 14.1%, 30.6%, 33.3% and 22.7% less postural sway, respectively. Significant PRD change from a supine to standing position was observed between TCG and CG (p < 0.05). A significant correlation between PRD change (from supine to standing) and years of Tai Chi practice was observed (r = 0.80, p < 0.05). The findings of this study support the positive effects of Tai Chi exercise on balance control under different conditions. Long-term Tai Chi exercisers also demonstrated different reflex modulation from a supine to standing position, and long-term Tai Chi practice may lead to a change of PRD modulation as neuroadaptation.

    Effect of Tai Chi on Body Balance: Randomized Controlled Trial in Elderly Men with Dizziness

    The purpose of this study was to assess the effect of 18-week Tai Chi training on body balance in a dynamic trial among elderly men with dizziness. The study covered subjects aged 60 to 80 years. We identified 40 men who reported a history of dizziness. The subjects were recruited using direct mailings and a community information campaign. The participants were randomly assigned to either the exercise intervention (n = 20) or control group (n = 20). The Tai Chi group participated in an 18-week exercise class held for 45 minutes twice a week. Body balance was studied in two ways: using the "8 foot up and go test" (Rikli and Jones 2001) and using a Computer Posturographic System PE 90 (manufactured by Military Institute of Aviation Medicine in Warsaw and outfitted with Pro-Med modified software). The ability to perform specific tasks (maximal deflections in four directions) was measured on the posturographic platform. The variation in results obtained on the first and second date of tests in the experimental and control groups was confirmed statistically using four parameters, i.e. "8 foot up to and go test (H = 8.21;p = 0.003), forward deflection (H = 3.70;p = 0.050), backward deflection (H = 5.04;p = 0.024) and maximum sway area (H = 8.86;p = 0.002). Consequently, we found that the 18-week period of Tai-Chi exercises, with a frequency of twice a week for 45 minutes, is beneficial for dynamic balance, which is important for the reduction of fall risk factors among elderly men with dizziness.

    Effects of Tai Chi on the Protracted Abstinence Syndrome: A Time Trial Analysis

    While exercise has been shown to reduce the negative effects of substance withdrawal symptoms, no research has investigated if Tai Chi, a traditional Chinese exercise, has similar effects. Here, we observed the physiological effects of Tai Chi on protracted abstinence syndrome (PAS) in female heroin addicts by comprehensively inspecting their immune system function, complete blood count, hepatic function and renal function. To determine the psychological effects, we used the Hamilton Rating Scale for Depression (HRSD) and the rating scale of heroin withdrawal symptoms. We recruited 70 heroin-addicted young women beginning to undergo withdrawal and randomly assigned them into two groups: one group received one-hour Tai Chi exercise every two days (Tai Chi group, n = 36) and the other group did not (control group, n = 34). Thirty-three patients finished this six-month trial. Numerous significant physiological differences were observed between all heroin-addicted subjects (n = 70) and age-matched healthy individuals (n = 18), suggesting a deleterious effect of drug addiction. There were improvements for certain physical parameters between the Tai Chi group (n = 17) and the control group (n = 16), although the differences were not statistically significant. We observed a small significant difference in psychological effects near the 60-day mark between the two groups. Taken together, our results suggest that Tai Chi might have a positive effect on PAS, which future studies can confirm by using an expanded sample size, longer trial time, and more sensitive and specific indicators of psychological and physiological health.

    The Effect of Tai Chi on Reducing the Risk of Falling: A Systematic Review and Meta-Analysis

    Tai Chi has frequently been used as a preventive measure against falling in at-risk populations. However, studies have yielded contradictory results, and literature reviews have considered only a small number of trials and have not addressed some key aspects, such as sources of heterogeneity and publication bias. This study includes 13 controlled trials published before June 2015 that analyzed the effectiveness of Tai Chi in fall prevention in populations of frail and at-risk adults. The effect measure used in this meta-analysis was absolute risk reduction (ARR) with a 95% confidence interval (CI). According to our findings, practice of Tai Chi significantly prevents the risk of falling (ARR, 0.10; 95% CI: 0.06, 0.15). The heterogeneity of results across the trials was low, with a reduced risk of publication bias, and no significant effect differences were observed between studies comparing Tai Chi with other interventions or non-treatment. We therefore conclude that Tai Chi is more effective than other measures, or no intervention, for fall prevention in at-risk populations. Further research is warranted to analyze the consequences of falls and to study the episodes rather than the cases of falls.

    Qigong and Tai Chi on Human Health: An Overview of Systematic Reviews

    Qigong and Tai chi are traditional methods of physical and mental training and exercises in East Asia. Their health-promoting effects against various diseases have been studied for a long time, and they have been the subject of many clinical trials and systematic reviews (SRs). The present study aimed to comprehensively evaluate all published SRs on Qigong and Tai chi and to summarize the supporting evidence. The following databases were searched: PubMed, Embase, Cochrane Library, China National Knowledge Infrastructure, Research Information Sharing Service, and Korean Studies Information Service System. The selection and extraction processes were performed by two independent reviewers, and a qualitative synthesis was conducted. There were 58 SRs of randomized controlled trials on Qigong and Tai chi. There have been many studies on patients with cardiovascular diseases and different cancers, and a number of other studies in which mobility, quality of life, blood lipids, and blood pressure were outcome measurements. Thus, Qigong and Tai chi for various diseases and medical conditions have been accumulated. Based on current evidence, the number of publications of Qigong and Tai chi-related articles showed an increasing trend, and most of them were performed in China. Qigong and Tai chi have shown beneficial effects in different age groups and health conditions, including decreasing blood lipid level, reducing blood pressure, facilitating mobility, preventing falls, and improving overall quality of life.

      Effect of home-based Tai Chi, Yoga or conventional balance exercise on functional balance and mobility among persons with idiopathic Parkinson’s disease: An experimental study

      Background: Individuals with Parkinson’s disease (PD) invariably experience functional decline in a number of motor and non-motor domains affecting posture, balance and gait. Numerous clinical studies have examined effects of various types of exercise on motor and non-motor problems. But still much gap remains in our understanding of various therapies and their effect on delaying or slowing the dopamine neuron degeneration. Recently, Tai Chi and Yoga both have gained popularity as complementary therapies, since both have components for mind and body control.

      Objective: The aim of this study was to determine whether eight weeks of home-based Tai Chi or Yoga was more effective than regular balance exercises on functional balance and mobility.

      Methods: Twenty-seven individuals with Idiopathic PD (Modified Hoehn and Yahr stages 2.5–3) were randomly assigned to either Tai Chi, Yoga or Conventional exercise group. All the participants were evaluated for Functional Balance and Mobility using Berg Balance Scale, Timed 10m Walk test and Timed Up and Go test before and after eight weeks of training.

      Results: The results were analyzed using two-way mixed ANOVA which showed that there was a significant main effect for time as F (1, 24) =74.18, p=0.000, ηp2=0.76 for overall balance in Berg Balance Scale. There was also significant main effect of time on mobility overall as F(1, 24) =77.78, p=0.000, ηp2=0.76 in Timed up and Go test and F(1, 24) =48.24, p=0.000, ηp2=0.67 for 10m Walk test. There was a significant interaction effect for time×group with F(2, 24) =8.67, p=0.001, ηp2=0.420 for balance. With respect to mobility, the values F(2, 24) =5.92, p=0.008, ηp2=0.330 in Timed Up and Go test and F(2, 24) =10.40, p=0.001, ηp2=0.464 in 10m Walk test showed a significant interaction. But there was no significant main effect between the groups for both balance and mobility.

      Conclusion: The findings of this study suggest that Tai Chi as well as Yoga are well adhered and are attractive options for a home-based setting. As any form of physical activity is considered beneficial for individuals with PD either Tai Chi, Yoga or conventional balance exercises could be used as therapeutic intervention to optimize balance and mobility. Further studies are necessary to understand the mind–body benefits of Tai Chi and Yoga either as multicomponent physical activities or as individual therapies in various stages of PD.

      Complementary and Alternative Medicine: The Perspective of a Cancer Patient

      Now growing at a rate of over 5% per annum, the $3 billion ‘alternative health therapies’ business is now positioned in the top ten growth industries in Australia. With poor regulation of both therapeutic goods and the unregistered therapists who promote them, cancer patients may well be putting their health at risk when they place their faith in many so-called ‘natural’ or ‘traditional’ treatments. With a focus on what complementary therapists refer to as ‘energy medicine’ and ‘nutritional medicine’, this chapter explores the risks and benefits of some of the more popular alternative health-care choices. While investigating their histories, it outlines what influences cancer patients to try these unproven therapies, and the conflict and contrast in information relating to the claims made for them and the conclusions of evidence-based research. Although there are a number of complementary therapies that are of benefit to some patients, both during and after their cancer treatments, ‘natural’ does not always equal ‘safe’, may be expensive and may even compromise their health. More patients now want a greater say in their choices of treatment, and selecting complementary therapies that may help is another of the many challenges faced in trying to make informed choices, as we navigate along our individual roads on our journeys to recovery.

      Chapter 7: Natural Healing: Qi Gong, Tai Chi and Yoga

      For thousands of years, a unique system of health promotion using natural means, viz. food, life style and exercises, has developed, matured and has been widely practised in China and India. “Natural Healing” might not be the best term to describe this system of health promotion but it is difficult to create more appropriate terminologies. One direct translation of this system of health promotion could be the maintenance or promotion of wellness. Natural Healing for Traditional Chinese Medicine (TCM) comprises the three components of physical, physiological and psychosocial harmony which are all interlinked. The ancient healers have worked out varieties of means to help bolstering the state of harmony.

      Natural Healing in Chinese Medicine: Qi Gong and Tai Chi

      Exercises involving stretching and control breathing are among the most important measures for the maintenance of health and natural healing according to the ancient Chinese Classic on Medicine. Meditation is another essential component in the pursuit of health.

      Qi Gong and Tai Chi are the most common body training practices requiring the tripartite combination of stretching, controlled breathing and meditation. The physiological basis of the practices are described in this chapter. While carefully planned clinical investigations have been performed to show the physiological benefits of Qi Gong and Tai Chi, the perfect scientific explanations are still remote. This tripartite combination of stretching, control breathing and meditation can help stimulate infrequent neurological activities and its versatility will encourage this practice in all health-conscious individuals.
