Dupuytren disease (DD) is a fibroproliferative disorder that originates at the palmar fascia and results in flexion contracture of the digits. The gold standard treatment for DD has been surgery. Surgical treatment of DD can be done with various methods, from percutaneous needle fasciotomy to dermatofasciectomy. The most commonly used surgical treatment is limited fasciectomy (LF). The potential complications of limited fasciectomy include long recovery time, scarring, neurovascular injury, and tendon injury. Minimally invasive treatment for DD, which is simple and allows for rapid return of function, has gained popularity amongst both patients and surgeons. Currently, minimally invasive treatments for DD include percutaneous needle fasciotomy (PNF) and collagenase clostridium histolyticum (CCH). In this review, we provide an overview of the minimally invasive treatments for Dupuytren disease and summarize the current evidence regarding these minimally invasive treatments.

Kienbock’s disease remains a condition of uncertain etiology and our understanding about the pathogenesis is still evolving. However, a lack of consensus on the treatment is the most striking as there are wide array of treatment options described ranging from ‘doing nothing’ to a free vascularized bone graft reconstruction of the lunate. Furthermore, most treatment modalities report equivalent success rate but cumulative evidence is lacking. This ‘black hole’ in the literature presents difficulties to surgeons treating the condition and even patient’s confidence with the treatment. All the treatment options described reliably relieve pain, thus improving function and patient satisfaction. However, the disease process is not reliably altered by any of the treatment options described till date. The operative treatment chosen depends on the stage of the disease, ulnar variance, age of the patient and surgeon preference. The treatment options in early stage (before lunate collapse) aim to preserve the lunate while once the lunate is collapsed its removal is performed. The lunate preserving operations basically are either decreasing the load on the lunate to possibly permit its revascularization or are the procedures which are aimed at augmenting the vascularity of the lunate. Radius shortening osteotomy is the most popular treatment in the early stages especially in the patients’ with negative ulnar variance and the proximal row carpectomy is most popular in the advanced stages of the disease. Authors present their algorithm of management of Kienbock’s disease and discuss the various treatment options described in the literature in an attempt to find the apt in 2020.

Upper extremity trauma is one of the most common types of injuries, accounting for 20 to 40 percent of injured patients presenting to the emergency department. In severe upper extremity injury, the likelihood of secondary procedures to improve function and resolve outstanding clinical problems is high. Secondary procedures are performed later than initial primary surgery, usually after wound healing and with a discernible time gap of days, weeks or occasionally longer. They should be considered as separate procedures from the primary surgery, with due diligence given to planning for it. When approaching secondary procedures, the key principles are anchored in early preparation, patient engagement, optimal timing and sequencing, reviewing and considering alternatives, and finally knowing when to stop. Although secondary procedures after upper extremity injuries can be a complex undertaking, the careful application of these principles and considerations will optimise outcomes.

Background: The purpose of this retrospective study was to report and compare the outcomes of the MAIA^® prosthesis and trapeziectomy for trapeziometacarpal osteoarthritis.

Methods: We included 92 consecutive patients (8 men, 84 women) with a mean age of 57 years (range 44–75 years) who underwent trapeziectomy (44 patients) or trapeziometacarpal joint replacement with a MAIA^® prosthesis (48 patients) for painful osteoarthritis. All patients were evaluated (pain, range of motion, strength, function, X-rays) by an independent examiner.

Results: The two groups of patients had comparable preoperative characteristics. At the mean follow-up of 9 years (range: 8–10), we found a significant reduction in pain levels for each group. In the final data review, there were no significant differences in terms of pain levels, grip strength, thumb active motion and the Quick Disabilities of the Arm, Shoulder and Hand score between the two groups. Pinch strength and the work performance score on the Michigan Hand Questionnaire were significantly better in the MAIA^® prosthesis group. The MAIA^® group had a shorter postoperative recovery time of 6 weeks and fewer patients required physiotherapy. Postoperatively, the thumb column length was significantly less in the trapeziectomy group. In this group, we found a significant decrease in the trapezial cavity height between the immediate postoperative evaluation and the final assessment, with three patients having painful scaphometacarpal impingement. Two patients required surgical revision for symptomatic meta-carpophalangeal joint hyperextension. In the MAIA^® group, we found no implant subsidence, loosening, dislocation or fracture. None of the implants were revised.

Conclusions: From this study, we found that the both procedures can be used as a surgical treatment for trapeziometacarpal osteoarthritis. The MAIA^® prosthesis is a useful alternative to trapeziectomy and appears to be a reliable and effective implant in the medium- to long-term.

Background: The Michigan Hand Outcomes Questionnaire (MHQ), a self-reported questionnaire for patients with hand disorders, has been widely used globally. It has been cross-culturally adapted into different languages across all continents. Aims of this study were to translate the MHQ into Malay language and to evaluate its reliability and validity compared with Disabilities of the Arm, Shoulder and Hand (DASH) questionnaire in a Malay-speaking population.

Methods: The MHQ was cross-culturally adapted into a Malay version based on the guidelines. A pre-testing involving thirty patients with hand disorders was performed to assess whether it was comprehensible to the target population. One hundred patients with hand disorders were recruited in this study to answer the MHQ and DASH questionnaires. The MHQ was tested twice with an interval of two weeks in between. Statistical analysis was performed to assess the reproducibility and internal consistency via the test-retest method and Cronbach’s alpha calculation, respectively. The association between MHQ and DASH questionnaire was assessed with Spearman’s correlation calculation.

Results: In the pre-testing, twenty-six patients (86.7%) understood all the questions in the Malay version of MHQ. The test-retest analysis showed a good reliability across the duration of two weeks with the intraclass correlation coefficient of all subscales ranging from 0.925 to 0.984. Cronbach’s alpha values of the Malay version MHQ ranged from 0.82 to 0.97, indicating a good internal consistency. Spearman’s correlation factor of the MHQ in comparison with DASH showed a fair to moderately strong correlation with the values ranging from 0.513 to 0.757.

Conclusions: The Malay version of MHQ was successfully translated and culturally-adapted with excellent reliability (reproducibility and internal consistency) and good construct validity.

Background: There is no consensus as to which is the best way to fix unstable extra-articular phalangeal fractures. We performed a literature review to determine outcomes using intramedullary cannulated compression screws.

Methods: PubMed, Medline and Embase databases were searched for English language articles reporting the use of one or more intramedullary screw(s) for fixation of an acute extra-articular phalangeal fracture in adults that reported outcome data.

Results: A total of 62 abstracts were identified, of which 14 full-text articles were eligible for full-text screening for the inclusion criteria. Of these 14 articles, seven articles were included in the final study. Data on 146 phalangeal fractures were aggregated with an average patient age of 36 (18–84). 89% (130/146) fractures were of the proximal phalanx, the remainder of the middle phalanx. Surgical technique was mostly a minimally invasive technique under fluoroscopy guidance using an anterograde technique. One paper described a retrograde method. Screw diameter varied from 2.2 mm to 3.2 mm. Where the radiographic union was reported, all fractures united. Range of motion averaged 231° (range 95°–295°) in four studies. Return to work was reported as 6.4 weeks (range 0.9–16.7 weeks) in one study. Different quality-of-life scores were reported by the studies (DASH 3.52 (range 1–45), QuickDASH score of 3.9 and DASH score of 3.9). Complications rate was 5% (8/146) including 5 major complications and 3 minor in the pooled results.

Conclusions: This literature review demonstrates that fixation of extra-articular fractures of the proximal and middle phalanges with cannulated compression screws is a safe and successful surgical treatment with faster mobilization and return to work with fewer complications, including stiffness, than have been reported with plates and screws and percutaneous K-wires. This application is limited to transverse and short oblique extra-articular fractures.

Background: The purpose of the present study was to assess the biomechanical strength and properties of a modified Krackow technique for side-to-side tendon repair with a short overlap length.

Methods: The flexor digitorum superficialis, flexor digitorum profundus, and flexor pollicis longus muscles were harvested from 10 fresh frozen cadavers. Overall, 60 tendon repairs were divided into four groups based on the suture technique: modified Krackow technique repair (KT); weave suture repair (WS); mattress suture repair (MS); and composite technique repair (CT), a combination of the modified Krackow and weave suture techniques. Single loading mechanical tests were performed, and the results for each suture technique were compared.

Results: Ultimate loads for KT, WS, MS, and CT were 155 ± 45 N, 122 ± 18 N, 92 ± 31 N, and 163 ± 22 N, respectively. KT and CT had significantly higher ultimate loads than the other groups. However, the difference between the KT and CT groups in terms of ultimate load was not significant.

Conclusions: Based on the results from the single loading tests, the use of the modified Krackow and composite techniques appeared to provide stronger fixation than that with the use of the weave and mattress sutures with a short overlap length.

Background: The DePuy Synthes variable angle LCP^® Volar RIM Plate has been designed with its contour to overhang volar rim fragments of the distal radius to prevent secondary displacement after fracture fixation. Therefore, RIM potentially contributes to a risk of flexor tendon rupture due to its plate prominence over the watershed line. This is a retrospective cross-sectional study that aims at describing the occurrence of complications by utilizing RIM as well as three standards VLPs to evaluate the impact of plate design on flexor tendons.

Methods: A total of 84 patients with a volar locking plate fixation were analyzed retrospectively (Far-distal; RIM: 14 cases, Juxta-articular; AcuLoc2 (ACUMED): 20 cases, Dual-Loc V7 (MEIRA): 25 cases, Extra-articular; VA-TCP (DePuy Synthes): 25 cases). Tendinous problems were evaluated with a reported adverse event and flexor tendon appearance examined by utilizing ultrasonography and direct observation during hardware removal. Plate prominence was graded according to Soong’s classification and the distance to flexor tendons was measured to describe the risk that each plate damage the flexor tendons.

Results: The mean follow-up period was 13.0 months. All RIM plates were placed in grade 2 (85.7%) and grade 1 (14.3%). Symptomatic and asymptomatic tendinous problems were barely reported in RIM. The examination of ultrasonography and direct observation during hardware removal showed that tendons smoothly slid without attrition on the curved plate surface of RIM despite their adjacent appositions came from its high and distal plate prominence.

Conclusions: Smooth profile of RIM showed an ability to decrease the potential risk of flexor tendon damage regardless of its prominence over the watershed line. RIM can be applied more to challenging far distal fractures when used judiciously and surgeons need to be even more vigilant for symptoms of tendon irritation as usually recommended without being coward more than necessary.

Background: Primary treatment of trigger digits is conservative including stretching, night splinting and combination of heat and ice. When these methods fail, invasive methods such as corticosteroid injection, percutaneous release and open surgery are used. The purpose of this study is to compare the efficacy of two outpatient methods of percutaneous trigger finger release (PTFR) and corticosteroid injection (CI).

Methods: This study is a randomized clinical trial that was performed with 6-month follow up. A total of 83 patients with trigger finger treated either with corticosteroid injection (n:40) or percutaneous release of the A1 pulley (n:43) were enrolled in this study. Demographic data were recorded before intervention. Pain score (VAS criterion), disease stage (Quinnell criteria), patient satisfaction and complications such as paresthesia, scarring, and stiffness (decrease in the range of motion) were recorded after the intervention. We used SPSS program (statistical package for the social science SPSS version 16) to perform the analysis.

Results: There were 18 male (21.7%) and 65 female (78.3%) patients, whose mean age was 52.54 ± 11.45 (28–85) years. There was a significant difference between the degree of pain at the time of the third, sixth weeks and sixth months in two groups. The degree of pain was lower in the CI group in the third and sixth weeks but it was lower in the PTFR group in the sixth month. Satisfaction of the patients in the sixth month was significantly higher in the PTFR group. The incidence of stiffness was also significantly lower in the PTFR group in the sixth month.

Conclusions: Patients in PTFR group had greater recovery and satisfaction level and lower recurrence rate and pain. Therefore PTFR may be used as a substitute for CI in the treatment of trigger finger from the beginning especially in patients who do not want to have open surgery.

Background: The Kapandji scale has 10 levels and includes abduction, rotation and flexion movements. Assessing children with thumb hypoplasia, this scale is poorly understood and confusing. The objective of the study is to evaluate an opposition scale simpler and understandable for children.

Methods: It is a validation study of a simplified scale to qualify the opposition of the thumb, in order to asses only the abduction - rotation but not the flexion of the metacarpo-phalangeal (MP) or interphalangeal (IF) joint. The proposed classification goes from 0 to 3 degrees, where 0: No opposition, 1: The thumb touches the middle phalanx of the 2nd finger, 2: the thumb touches the pulp of the 2nd finger and 3: the thumb touches the pulp of the 5th finger. 10 patients with thumb hypoplasia were analyzed by 9 hand surgeons and 9 orthopedic surgeons.

Results: The intraclass correlation coefficient shows a very good inter-observer reliability with a kappa of 0.991 (p = 0.000). When correlated by groups, the finding were very good between the hand surgeons group k = 0.980 (p = 0.000) and the orthopedic surgeons group 0.974 (p = 0.000). At 6 weeks a new evaluation was made, the intra-observer reliability was excellent k = 0.995 (p = 0.000).

Conclusions: The proposed scale for the evaluation of the abductionrotation of the thumb is validated and useful in the evaluation of the results of a thumb opposition transfer with a good inter-observer and intra-observer reliability between orthopedic and hand surgeons.

Background: Lateral humeral condylar fractures often heal with some residual elbow deformity. However, details of angulation or tilting angle of the lateral condyle after the fracture have not been evaluated so far.

Methods: Between 2008 and 2016, we followed up 80 mild fractures of the lateral humeral condyle for more than a year. Thirty fractures were treated by open reduction and internal fixation (ORIF) with Kirschner wires. Fifty cases were treated with a long arm splint for 3 weeks (Fig. 1). The average age of the patients at the time of the injury was 5.5 years. The humerus-elbow-wrist angle (HEWA), Baumann’s angle (BA), and tilting angle (TA) were measured on the radiographs. The active range of motion (ROM) was clinically assessed at unaffected and affected sides at the final follow-up.

Results: No significant differences were detected between the sides about TA or ROM at the final follow-up. However, HEWA/ BA showed more significant loss of correction. There were significant differences in BA at the affected side between the ORIF and splint groups.

Conclusions: Cubitus varus deformity after lateral humeral condylar fracture is not accompanied by a change in TA or ROM, unlike the deformity after supracondylar or distal epiphyseal fracture of the humerus (Fig. 2). Operative treatment to precisely correct and fix the lateral condylar fracture still retained some cubitus varus deformity, although it might lessen or prevent the deformity when compared to conservative treatment with a splint.

Background: Ultrasonographically measuring the median nerve cross-sectional area (MN-CSA) is a useful and complementary method for diagnosing carpal tunnel syndrome (CTS). This study investigated the usefulness of the median nerve transverse diameter (MN-TD) as a simpler and easier technique than MN-CSA for diagnosing CTS severity.

Methods: We examined 145 hands from 101 patients with CTS and 114 age- and gender-matched control hands. Based on the electrodiagnostic findings, the severity of CTS was divided into minimal, mild, moderate, severe and extreme. An ultrasonographic assessment of MN-TD and MN-CSA was performed to measure the maximal values around the carpal tunnel inlet level. The values were assessed for comparisons between the CTS and control groups and for the evaluation of differences by CTS severity.

Results: The ultrasonographic measurements of MN-TD and MN-CSA were significantly increased in the CTS group compared to the normal group. Furthermore, both measurements significantly increased with increasing CTS severity. A receiver operating characteristic curve (ROC) analysis showed that participants with MN-TD > 7.2 mm had CTS with 88% specificity and 74% sensitivity, and those with MN-CSA > 12.0 mm² had CTS with 86% specificity and 84% sensitivity.

Conclusions: Ultrasonographic measurement for MN-TD may be useful for diagnosing CTS patients and predicting the severity.

Background: Dupuytren’s disease (DD) is well known as a fibromatous disorder of the palmar aponeurosis. Although there is a large body of literature on the etiology of DD in Europe, there have been few studies in Japan. The purpose of our study was to investigate the etiology and risk factors of DD in a large population in Japan.

Methods: The subjects were voluntary participants from the 2014 Health Promotion Project, and 1,112 individuals were included (421 men and 691 women; mean age = 54.2 ± 15.3 years) in this study. The severity of DD was assessed using the Meyerding classification. All participants completed a questionnaire on age, sex, lifestyle, and occupations. All participants also completed the Short Form Health Survey (SF-36) to investigate the QOL. Fasting venous blood specimens were taken for biochemical analysis.

Results: DD was found in 44 participants (3.9%). The prevalence was 8.3% among the men and 1.3% among the women. The prevalence in these older than 60 years was 7.7% (men; 18.5%, women; 2.4%). There were 17 participants that had DD of both hands. Multiple digits were affected in 9 participants. The ring finger was the most affected finger (71.2%), followed by the little finger (16.4%), and middle finger (12.3%). In Meyerding classification, 39 cases were stage 0, one case was stage 1, and four cases were stage 2. The multivariate logistic regression analysis revealed that age, sex, smoking, and occupation were significantly associated with DD. There was no significant association between DD and other parameters. When associations between DD and the SF-36 subscales were analyzed, there were significant associations with physical functioning, physical role functioning, and mental health.

Conclusions: Our study is the largest in Japan to date. These results will provide very useful data to aid understanding of DD.

Background: The induced membrane technique is now commonly used for large diaphyseal bone defects. Recently, several papers reported using the induced membrane technique for hand surgery. We applied this technique with some modifications to treat osteomyelitis of the phalanges.

Methods: This study included six men and one woman with a mean age of 56 years. The causes of osteomyelitis included animal bite (n = 3), trauma (n = 3), and an indwelling needle (n = 1). Two-staged surgeries were performed, including an initial stage with radical debridement of the infected tissue and placement of a cement spacer into the bone defect. Four weeks after the first stage, a bone graft was performed. A bone block with cortex was harvested from the iliac crest or radius, and costal cartilage was used for proximal interphalangeal (PIP) joint arthroplasty in two cases. Grafted bones were fixed with a mini screw or an external fixator.

Results: In all cases, the infection subsided, and bone union was obtained within two to three months. No absorption of the grafted bone was observed. In the two cases with PIP joint defect, joint motion without pain was preserved at 56° and 26°.

Conclusions: A short interval between the two surgical stages of the induced membrane technique could be advantageous for patients in terms of time and financial burden and early rehabilitation of movement. Cortico-cancellous bone grafts were able to maintain bone length and stability with screw fixation. In the cases with PIP joint defects, instead of arthrodesis, we performed PIP arthroplasty using costal cartilage, eventually obtained some motion without pain. The induced membrane technique was useful and technically feasible for treating osteomyelitis in the hand, and secondary joint reconstruction was possible to obtain some motion.

Background: Hemi-hamate arthroplasty is one of the treatment options for dorsal proximal interphalangeal joint fracture dislocation. Many studies reported favorable outcomes. However, some long term studies demonstrated the degenerative change of PIP joint. The articular mismatch of the hemi-hamate autograft might play an important role of this complication.

Methods: We studied the anatomy of distal articular surface of the hamate bone in embalmed hamate bones. The anatomy of distal articular surface and dimensions of the hamate bone were measured and recorded.

Results: Seventy hamate bones were dissected out from embalmed cadavers and included in this study. The mean angle of the 4^th metacarpal articulation was 85.54 degrees (SD = 3.53) and mean angle of the 5^th metacarpal articulation was 95.51 degrees (SD = 3.57). The inter-articular ridge was approximately 5 degrees radial inclination.

Conclusions: The inter-articular ridge of distal articular surface was approximately 5 degrees radial inclination. In order to minimize the graft mismatch, we recommend making the bone cut with the saw blade tilted to radial side 5 degrees to achieve better alignment of inter-articular ridge of the graft.

Background: Osteoarthritis of the thumb base is the second most prevalent arthritis of the hand. Management is primarily conservative, consisting of analgesia, splinting, physiotherapy, and steroid injections. Surgery is considered when conservative measures fail.

Methods: The primary objective was to assess the safety and efficacy of the surgical interventions and therein, evaluate whether any superiority exists among the available interventions. Efficacy was evaluated by examining four parameters: pain, function, range of movement and strength of the joint postoperatively. Safety was determined by comparing the rate and severity of postoperative complications. A systematic search of MEDLINE (2014–2019), EMBASE (2014–2019), CINAHL (2014–2019) and CENTRAL (2014–2019) databases was carried out. Abstracts were screened for relevant studies. Randomised controlled trials were only considered.

Results: Eight studies were included in the quantitative synthesis. The procedures evaluated are: Trapeziectomy (T), trapeziectomy with ligament reconstruction (T + LR), trapeziectomy with ligament reconstruction and tendon interposition (T + LRTI), trapeziectomy with allograft suspension (T + ALS) and joint arthrodesis (A). Low-moderate quality evidence suggests that T + LRTI yields better range of movement (palmar abduction) when compared with (T) alone; (SMD 0.61, 95% CI 0.22 to 1.00, random-effects, p = 0.002). Comparing adverse events showed that arthrodesis carries a greater risk of adverse events when compared with T + LRTI; (RR 0.33, 95% CI 0.17 to 0.61, random-effects, p = 0.0005). In addition, T + LRTI is preferred over arthrodesis by patients (OR 0.29 95% CI 0.09 to 0.95; p = 0.04). This difference was no seen in the other comparison groups.

Conclusions: It is difficult to declare with any degree of certainty which procedure offers the best functional outcome and safety profile. Results suggest T + LRTI yields good postoperative range of movement. Arthrodesis demonstrated an unacceptably high rate of moderate-severe complications and should be considered with careful consideration.

Background: Digital island flap remains among the most useful types of providing soft-tissues coverage for defect on the fingertip accompanied with underlying structures exposure. Nevertheless, its trends of functional and aesthetic issues such as the limited length of advancement and the limited flap size are the essential disadvantages. The main objective of the study was to assess the clinical effectiveness of dorsal branch skin flap of proper palmar digital artery with distal pedicle in repairing of fingertip soft tissue defect accompanied with underlying structures exposure.

Methods: This is a 1-year prospective study, in which 21 patients (24 fingers) with traumatic fingertip skin and soft tissue defects had undergone emergency repair with dorsal skin flap of proper palmar digital artery with distal pedicle. The starting point of the dorsal proper palmar digital artery which is at the connection of distal interphalangeal joint with the digital midline was selected as the rotation point of the surgical skin flap, with an incision area of 1.1 cm × 1.4 cm–2.7 cm × 2.0 cm, the pedicle of 0.8–1.7 cm and disposition of 90°–120° to cover the fingertip.

Results: All flaps had completely healed postoperatively. In 2 cases, tension blisters of the flap and partial necrosis were observed, which survived after change in dressing and rational administration vasodilator medication. Patients were followed up to 6 months. Favorable appearance and function were restored, and satisfactory recovery of the sensory functions of the fingers had been achieved. The two-point discrimination of skin flap with anastomosis of nerve could reach 4.5–9.4 mm (mean, 6.80 mm).

Conclusions: Repairing the fingertip skin and soft tissue defect of 2^nd–5^th finger via primary surgical repair with dorsal distal pedicle or proper palmar digital artery skin flap is a good technique. The high survival rate of the flap, the favorable flap’s appearance, and the satisfactory recovery of the sensory functions of the fingers are among the advantages of this reconstructive technique.

Closed traumatic rupture of forearm flexor muscles has been reported rarely. Previous reports have included ruptures of the flexor digitorum profundus, flexor digitorum superficialis, flexor pollicis longus, flexor carpi radialis, pronator quadratus and the palmaris longus. We report a patient with a closed traumatic rupture through the muscle belly of the flexor carpi ulnaris and summarise the published literature on ruptures involving the forearm flexor muscles. Overall, conservative treatment can result in excellent outcomes. Early surgical intervention is recommended in patients with nerve involvement and compartment syndrome and delayed reconstruction may be considered for patients with functional deficits.

Acute plastic deformation of long bones is more common in young children. We report a case of an acute plastic deformation of a pediatric radius via magnetic resonance imaging (MRI) evaluation. A 15-year-old boy fell on landing after a jump while practicing soccer, which injured his right forearm. He was diagnosed with a radial neck fracture and a medial epicondylar fracture of the humerus on the basis of plain radiograms. MRI was additionally performed and showed abnormal shadows indicating intramedullary bleeding at multiple bamboo-joint-like deformity sites of the radius. Surgery was performed and injury completely healed. Acute plastic deformation of long bones was often diagnosed by simple radiographic imaging. To our knowledge, there has been no previous reports of plastic deformation evaluated by MRI. If bone plastic deformation is missed, functional impairments such as limited range of motion remain; thus, an early diagnosis of acute bone plastic deformation by performing MRI is recommended.

We report two pediatric cases of radial nerve palsy caused by supracondylar fracture of the humerus requiring nerve exploration. The children had comparable conditions, palsy types (complete motor and partial sensory) and radiographic findings. The fracture in the first case was managed conservatively with closed reduction and percutaneous pinning but, while the patient eventually recovered from the partial sensory palsy, her motor palsy remained unchanged. In the second case, ultrasound assessment of the nerve prior to fixation indicated that surgical exploration was needed as it revealed tethering on the edge of the proximal fracture fragment. The nerve was released during an open reduction and the patient subsequently recovered from both sensory and motor palsies. Ultrasonography proved essential in the initial clinical assessment by determining how to proceed. We recommend primary nerve exploration when ultrasound findings show entrapment or tethering of the radial nerve.

Lunate coronal fracture is a rare injury, associated with impact along the capitate and lunate axis; neutralization of this axial force is important for satisfactory patient outcomes. This report presents a case of a 51-year-old man with a lunate coronal fracture associated with coronal fractures in the proximal carpal bones and volar and proximal dislocation of the distal carpal row. Open reduction and internal fixation with a dorsal distraction bridge plate were performed. Solid bony union was achieved at the fracture site and the patient resumed full activities without difficulty. Use of distraction bridge plate fixation can support treating this type of injury, in particular, in cases where the lunate fracture is difficult to treat with conventional osteosynthesis techniques; the presented approach provides good short-term results.

Wrist arthrodesis has been used successfully for the management of severe wrist flexion deformity when soft tissue procedures would not provide adequate correction. However, in athetoid type cerebral palsy which has a component of involuntary movement, the outcome of wrist arthrodesis has not been discussed much. We present our experience in 2 athetoid type cerebral palsy patients who underwent wrist arthrodesis due to severe involuntary movement of the wrist. One patient had a nonunion and both patients had unexpected aggravation of involuntary movement in the adjacent joints. Secure fixation using a pre-contoured plate is necessary and preparation for iliac bone grafting should be considered as proximal row carpectomy is usually not necessary in these patients. In addition, although single-event, multi-level surgery is advocated for patients with cerebral palsy, potential additional procedures for the adjacent joints should be discussed preoperatively because unexpected aggravation of involuntary movement of adjacent joints can occur after stabilization of the wrist.

A 68-year-old man with a right distal radius fracture treated with volar locking plate fixation previously was unable to flex his right thumb and four fingers without experiencing any adverse event. The flexor pollicis longus was reconstructed with a tendon graft using the lengthened flexor digitorum superficialis. The flexor digitorum profundus (FDP) of the ring finger was reconstructed with a tendon graft using the palmaris longus. The FDP of the index and little fingers was reconstructed using the interconnected tendon graft to the ring finger. Postoperatively, active flexion of all fingers and thumb was restored; however, he was unable to grasp thin objects because of the absence of full finger flexion. This is the first case wherein all nine flexor tendons being involved after volar locking plate fixation for a distal radius end fracture. We demonstrated a reconstructive procedure for long-standing multiple flexor tendon rupture after volar locking plate fixation.

Background: Traumatic loss of an index finger is offered twice the amount of compensation as compared to a loss of little finger. However, the little finger plays a pivotal role in power grip thus is underestimated in its importance. Our aim was to test our hypothesis that loss of the little finger will result in greater or equal loss of grip strength compared to loss of the index finger.

Methods: Grip strength in the power grip position was measured in 12 healthy volunteers using a JAMAR hand dynamometer. Grip strength of their dominant hand was recorded as a mean kg force of three attempts in three grip configurations 1) using all fingers 2) excluding the index finger and 3) excluding the little finger. Grip strength percentage compared to the full hand was calculated and statistical significance was investigated with a two-tailed T-test.

Results: Participants’ age varied from 19–64 years, with 4 males and 8 females. Mean full hand grip strength was 28.3 kg force; grip strength with index finger excluded was 65.8% and with little finger excluded was 66.2%. There was no significant difference in grip strength percentage when comparing index or little finger exclusion (p = 0.92).

Conclusions: We did not find a difference in power grip using a simulated model of index or little finger loss in a healthy volunteer cohort. This should be taken into account in traumatic loss and work compensation.
