This paper addresses the circuit implementation challenges resulting from the integration of a therapeutic clip in a magnetically maneuverable wireless capsule intended for colonoscopy. To deal with the size constraints typical of a capsule endoscope, an Application Specific Integrated Circuit (ASIC) has been designed specifically to habilitate the release of the therapeutic clip. The ASIC is a complete System on Chip (SoC) that incorporates a circuit for the low power release of the clip, thus overcoming the limitations of the power supply system. With a size of 14mm², the ASIC can be incorporated in practically any capsule endoscope, consuming only an idle-state power of 1.5mW.

Early prostate cancer detection and treatment are of major importance to reduce mortality rate. magnetic resonance (MR) imaging provides images of the prostate where an early stage lesion can be visualized. The use of robotic systems for MR-guided interventions in the prostate allows us to improve the clinical outcomes of procedures such as biopsy and brachytherapy. This work presents a novel MR-conditional robot for prostate interventions. The minimally invasive robotics in an magnetic resonance imaging environment (MIRIAM) robot has 9 degrees-of-freedom (DoF) used to steer and fire a biopsy needle. The needle guide is positioned against the perineum by a 5 DoF parallel robot driven by piezoelectric motors. A 4 DoF needle driver inserts, rotates and fires the needle during the procedure. Piezoelectric motors are used to insert and rotate the needle, while pneumatic actuation is used to fire the needle. The MR-conditional design of the robot and the needle insertion controller are presented. MR compatibility tests using T2 imaging protocol are performed showing a SNR reduction of 25% when the robot is operational within the MR scanner. Experiments inserting a biopsy needle toward a physical target resulted in an average targeting error of 1.84mm. Our study presents a novel MR-conditional robot and demonstrated the ability to perform MR-guided needle-based interventions in soft-tissue phantoms. Moreover, the image distortion analysis indicates that no visible image deterioration is induced by the robot.

Robotic systems are being applied to medical interventions as they increase the operational accuracy. The proposed autonomous and ultrasound guided 5-DOF parallel robot can achieve such accuracy for needle biopsies, which particularly demand precise needle positioning and insertion. In this paper, the robot’s mechanical design, system identifications, and the design of its controller are explained. A torque computed controller with gravity compensation and friction models, yielding a 0.678mm RMS position error for the needle tip, was used. A novel method was used for 3D space calibration of the images for detecting the volume of interest in the biopsy procedure by a multipoint crosswire phantom with parallel threads. The calibration technique had a validation RMS error of 0.03mm.

In tracked ultrasound systems, temporal misalignment between image and tracker data results in incorrect image pose. We present a fully automatic temporal calibration. We image a flat plate in water with a tracked probe undergoing periodic uniaxial freehand translation. Using robust line detection scheme, we compute temporal misalignment as difference between probe and corresponding image position. From 240 sequences, standard deviation was under 5 ms for standard imaging parameters. Source code is available in Public Library for Ultrasound Research, PLUS (www.plustoolkit.org).
